Enrollment Support

One conversation provides a comprehensive look at your benefits, from health to life insurance, from wellness plans to supplemental coverage. To speak with a Benefit Counselor from our dedicated Benefits Service Center, there are two options, over the phone or via email. Here’s how to reach one of our Benefits Counselor:

Before You Get Started

Make sure to take a look through this benefits website and your benefits brochure so that you fully understand all of the options available to you as an employee of BASS Medical Group. Once you have reviewed all of this information, follow the steps below to enroll using our online enrollment system.

How to Enroll

Once you have reviewed and selected the plans that are right for you and your family, click on the Enrollment Instructions area on the home page and then on the Enroll Now link. You will be connected to the Paylocity Self Service portal where you will be asked to enter your company ID, personal username and password.

You should have already logged into the Paylocity Self Service Portal and created your Username and Password. If you cannot recall your information, please click on the Need Help? and/or Forgot? links for assistance.

A Few Helpful Tips/Hints:
  • If you are enrolling your spouse or dependent children, please be prepared to enter their social security numbers and dates of birth or their enrollments could be delayed.
  • When enrolling, please select the Applications tab and then the BSwift Benefits link from the dropdown options.
  • Please remember to click through the entire enrollment site until you reach the Summary and Review section. Click the orange CONTINUE button at the top right hand side of the page. Next, scroll down to the bottom of the page and click on the “I agree, and I’m finished with my enrollment” box then click on the “Complete Enrollment” link. This will provide you with a summary sheet that will be emailed to you if the system has an email address for you. This will close out your transactions and provide you with an enrollment and payroll deduction summary. This will also signal the Benefits Service Center to process your time sensitive enrollment so that your information feeds to the carriers and payroll. Your enrollment is not complete until you finalize these tasks.
  • If you will be enrolling, you can expect to receive ID cards 2-4 weeks after your enrollment elections have been submitted to the carrier(s).
  • The cost of your health benefit elections can be found on the Paylocity benefits portal. The portion of the insurance premium paid by the employee is taken through (pre-tax) per paycheck deduction(s). The pre-tax option is not available for domestic partner coverage.

Accessing Your Account

When you are ready to enroll, click the button below to access our online enrollment system.

Once you launch your Web Pay Employee Self Service Portal, you will be able to access the Enterprise Web Benefits site by hovering over “Web Pay” in the upper left hand corner and then selecting the “Bswift Benefits” link from the dropdown options. You will not need a separate User ID or password when accessing the site via the Employee Self Service Portal.”

Making your Benefit Elections

From the Home Page, click on "Start Your Enrollment". The enrollment process consists of the following 4 steps/tabs. You will be taken through each tab to make changes or confirm your information on file and choose your benefits for the new plan year.

1. Employee (Personal Information)
2. Family (Family Information)
3. Enroll
4. Confirm

Verify your Personal Information

Before beginning your enrollment, please verify the accuracy of all of your personal information (e.g., address, DOB, etc.) If you need to make any changes, please do so in your Employee Self-Service portal. All updates will reflect on this page within 24 hours. You can still move on with your enrollment. Verify that all information is accurate. When done, check “I agree” at the bottom of the page and click “Continue."

Please Note: Any field that has an asterisk next to it is required.

Verify your Family Information

Please be sure to add all dependents that may be missing from the Family Information section before proceeding to the next section. To do this, click on "Add Dependents." When all of your family information is accurate, check “I agree” and click “Continue.”

Making Benefit Elections

From here, you can compare costs across all available plans and compare medical, dental and vision plan details side by side by selecting which dependents you would like to cover (the system will generate your coverage tier based on this) and clicking on the Compare Plans link. You can also review details of a specific plan by clicking on the View Plan Details link and then clicking on the name of the document that you wish to view. If you wish to add a new dependent at this time, click on the ‘Add Dependents’ button to be taken directly to the family tab to add the dependent. Once you have selected which dependents to cover, check the box next to the plan you would like to enroll in and click ‘Next’.

Almost Finished!

After selecting your benefits, you will be on the final review page. Review all of your benefit elections and covered dependents. If you wish to make any changes simply click on any one of the EDIT buttons. It will return you to the enrollment page.

Once you've completed your review, check the ‘I agree, and I’m finished with my enrollment’ and ‘Save my Enrollment’ at the bottom of the page. If you stop at any point before this step, your progress will be saved, but your enrollment is NOT complete.

You can send yourself an e-mail confirmation of your elections or print it for your records.

Before you Log Out, remember to check the ‘I agree, and I’m finished with my enrollment’ and ‘Save my Enrollment’ at the bottom of the page to complete your enrollment process.
